Hard Flaccid Syndrome: A Comprehensive Pelvic Health Approach

Hard Flaccid Syndrome (HFS) is a relatively rare but increasingly recognized condition characterized by a semi-rigid penis in the flaccid state, pelvic pain, erectile dysfunction, altered penile sensation, urinary symptoms, and psychological distress. Although its exact cause remains unclear, current evidence suggests that HFS is multifactorial and may involve pelvic floor muscle dysfunction, peripheral nerve irritation, altered blood flow, and central nervous system sensitization.

Symptoms may begin following minor penile trauma, vigorous sexual activity, excessive pelvic floor muscle tension, or prolonged straining. Chronic pelvic floor overactivity may place pressure on nearby nerves and blood vessels, contributing to pain, sensory changes, and impaired sexual function. Anxiety may further increase muscle guarding and contribute to a continuing symptom cycle.
