Balance Training for Posterior Tibial Tendon Dysfunction (PTTD): How Physical Therapy Can Help

Posterior Tibial Tendon Dysfunction (PTTD) occurs when the tendon that helps support the foot’s arch becomes weak or injured. It may contribute to foot pain, ankle instability, changes in arch support, and difficulty with walking or balance.

Balance training is an important component of physical therapy for PTTD. It develops proprioception—the body’s awareness of foot and ankle position—so the muscles can respond more effectively during standing, walking, and other functional movements.

Early activities may include weight shifting, tandem standing, and supported single-leg balance. As control improves, a physical therapist may add rhythmic stabilization, applying gentle forces while the patient maintains a stable position to train ankle stability, coordination, and neuromuscular control.

Exercise difficulty is progressed according to tendon irritability, strength, alignment, and balance ability. A broader program may also include strengthening, mobility work, gait retraining, footwear guidance, or custom orthotics when appropriate.
